Heidi
With over 30 years of experience in the disability sector, our Director Heidi brings deep expertise, insight, and a steadfast commitment to equality for all. Her professional knowledge is enriched by lived experience as a family member of a person with a disability, giving her a unique and compassionate perspective. Heidi embodies the values of kindness, integrity, and innovation, both personally and professionally.
She is acutely aware of the challenges faced by regional communities in accessing skilled, professional support and is driven to change this. Guided by principles of honesty, accountability, and fairness, Heidi has made it her life’s work to improve the lives of people with disabilities. She understands that while we all face barriers, we also possess strengths that make us vital contributors to our communities.
Under Heidi’s leadership, LiFT Supports has built a cohesive, multidisciplinary team dedicated to raising the profile, engagement, and treatment of people with disabilities. Together, they support each participant to enhance their daily lives, increase their economic participation, and be recognised as valued members of society.

Sarah K
With over 14 years in the disability sector and 6 years as a Support Coordinator, I bring a wealth of experience across paediatrics, complex cases, and life stage transitions. I’ve had the privilege of supporting individuals with a wide range of diagnoses, equipment needs, and goals, from early childhood through to aging adulthood. My background includes navigating SIL and SDA processes, coordinating hospital discharges, and working closely with multidisciplinary teams to achieve meaningful outcomes.
As Team Leader of the Geelong Support Coordination team, I’m proud to lead a group of passionate professionals who consistently go above and beyond for the people we support. Each coordinator brings a unique spark and deep commitment to their role, and I love mentoring them as they advocate for better outcomes and more empowered lives.
What drives me is the opportunity to understand each person’s story, because no two journeys are the same. I believe success lies in discovering what truly lights someone up and helping them build a life around it.
Outside of work, you’ll find me surfing at the beach, playing sport, or exploring nature with my partner and kids. It’s all about balance, connection, and living life fully, just as we support others to do.

Maree
Born and raised in our local community, I’ve been working in the employment and disability sector since 2010. Supporting people to achieve their goals is my passion, and I’m proud to contribute to the vibrant region I call home.
I joined LiFT Supports in June 2021, bringing a strong focus on efficiency, time management, and delivering positive outcomes. Known for my caring and approachable nature, I’m always happy to lend a hand or have a chat.
What I love most about LiFT is the chance to walk alongside individuals and families as they pursue their best lives. Collaborating with incredible locals and being part of a values-driven team makes every day rewarding.
Outside of work, you’ll often find me volunteering at the local footy club or whipping up something delicious in the kitchen.

Aaron
With over eight years of experience in the disability sector, I am deeply committed to supporting NDIS participants in achieving meaningful outcomes aligned with their personal goals. My academic background includes a Bachelor of Arts in Sociology, which has equipped me with a strong understanding of the importance of inclusive, person-centred support.
My lived experience further fuels my dedication to empowering individuals to live their best lives. I believe in fostering genuine connections, advocating for participant choice and control, and delivering services that promote independence and wellbeing.
Outside of work, I enjoy watching a wide range of sports and documentaries. I’m deeply committed to collaboration, lifelong learning, and the transformative power of shared insight, because real progress happens when we grow together and lift each other up.

Teneil
Teneil is known by those closest to her as kind, passionate, and deeply honest—qualities that shine through in every aspect of her work. With 17 years of experience across diverse roles in the disability sector, she brings a wealth of practical knowledge and heartfelt commitment to every participant she supports.
Her core focus has always been helping individuals achieve their goals, whether that’s through education, employment, housing, health, or other essential supports. Teneil is a skilled communicator who ensures participants feel heard, respected, and in control of their own journey. She takes pride in building strong, collaborative support teams that work together to deliver the best possible outcomes.
Teneil’s strength lies in her ability to coordinate complex supports with clarity and compassion. She understands that every person’s situation is unique, and she approaches each case with genuine care and determination. Her leadership and dedication make her a trusted guide for participants navigating the NDIS.

Melissa
With over five years of experience in the disability sector and a strong foundation in both medical and trade environments, I bring a practical, person-centred approach to every interaction. My work is grounded in real-world understanding and guided by empathy, respect, and a commitment to best practice.
I hold Certificate IV qualifications in Disability, Community Services, and Alcohol and Other Drugs, equipping me to support participants across a wide range of needs and circumstances. I’m experienced in navigating complex situations and helping individuals understand, activate, and maximise their NDIS plans, always in alignment with legislation, NDIS guidelines, and ethical practice.
I’m passionate about developing tailored, strengths-based supports that build capacity, promote independence, and reflect each participant’s unique goals and values. I prioritise authentic, collaborative relationships with participants and their families, ensuring the right mix of formal and informal supports is in place to enhance wellbeing and quality of life.
At the heart of my practice is a deep respect for lived experience. I listen actively, advocate fiercely, and work creatively to unlock what matters most to each person.
Outside of work, you’ll find me enjoying time with family and friends or exploring Geelong’s vibrant café scene, always on the hunt for a great coffee and good conversation.

Sarah
A proud Swan Hill local and one of LiFT Supports’ original team members, I’ve been working in the disability sector since 2008. I bring over 15 years of experience, a caring nature, and a practical, outcomes-focused approach to every participant I support.
Known for my efficiency, time management, and genuine commitment to helping others, I thrive on building strong relationships and creating tailored solutions that empower people to live life on their terms. I love collaborating with our incredible team and local networks to keep learning, growing, and achieving great outcomes together.
Outside of work, I’m a foodie and a bit of a homebody – happiest pottering in the garden, belting out tunes on a road trip (but still too shy for the dancefloor!). I value experiences over things, and I’m all about people and places. Fun fact: while I haven’t seen much of Australia, my passport gets plenty of use!
Oh and yes, I’m tall. But my teenage son is even taller!

Daniel
Daniel is a highly experienced Support Coordinator who’s been guiding participants through the NDIS since 2014. He understands that navigating the system can be overwhelming, even at the best of times, and is committed to making the process simpler, clearer, and more empowering for every individual he supports.
With a strong track record of working alongside multidisciplinary teams, Daniel is skilled at coordinating complex supports and achieving meaningful outcomes. He takes the time to explore each participant’s goals and ensures their funding is used effectively to build capacity, promote independence, and align with their values.
Daniel thrives in complex situations and brings a calm, solutions-focused mindset to every challenge. He’s passionate about helping people reach their full potential and making sure their voice is heard, because every person deserves to feel in control of their journey.

Peter
Peter brings nearly two decades of experience in the disability sector, with a strong foundation in social work and a deep commitment to participant-centred support. After graduating from RMIT in 2010, he worked as a support worker with DHS and later led a team at Villa Maria’s Acquired Brain Injury rehabilitation service from 2010 to 2015.
Since 2016, Peter has specialised in Support Coordination, working with Melbourne City Mission, Leisure Networks, and now proudly with LiFT Supports. Based in Geelong and part of the team since 2023, Peter is known for his calm, reliable approach and his ability to navigate the complexities of the NDIS with clarity and care.
Peter’s strengths lie in his communication, consistency, and broad experience supporting people with diverse disabilities. He’s passionate about doing what he says he’ll do, and doing it well. He values the strong team culture at LiFT Supports and the shared commitment to achieving great outcomes for participants.
Outside of work, Peter is a proud husband and father of two, and a self-confessed old-school heavy metal fan. Whether he’s coordinating supports or cranking up the classics, Peter brings authenticity, dedication, and a touch of rock ’n’ roll to everything he does.

Jodie
With a lifelong passion for helping others, I’ve worked across education, aged care, employment services, and now proudly with LiFT Supports. I bring warmth, reliability, and a genuine commitment to empowering people, whether it’s supporting participants to reach their goals or simply making sure they feel heard and valued.
My career began in retail, followed by roles as an Integration Aide in local schools, a cook in aged care, and seven years in employment services helping people build confidence and find meaningful work. Each role has deepened my love for community and strengthened my belief in the power of connection.
Outside of work, I’m a proud mum of three and grandmother to six. I enjoy patchwork, knitting, and crocheting, and love spending time on the river in our camper with family and friends. I’m also a passionate supporter of the Mallee Eagles – and always up for a good yarn and a bit of treasure hunting at the local op shops.

Jasmine
With over eight years of experience as an NDIS Support Coordinator and 16 years of lived experience as a parent navigating the disability system, Jasmine brings both professional expertise and personal understanding to every participant she supports.
Based in Geelong, Jasmine has been part of the NDIS journey since its pilot in 2013, adapting alongside its many changes and pathways. She works primarily with children and their families, but also supports older participants, always with a focus on building strong, trusting relationships and tailored support teams.
Jasmine is passionate about helping people find their ‘tribe’ – the right mix of formal and informal supports that align with their goals, values, and unique circumstances. She’s known for her empathy, clear communication, and solution-focused mindset, and she celebrates every win, big or small, alongside the people she supports.
For Jasmine, support coordination is more than a role – it’s a privilege. She understands the challenges, the triumphs, and the importance of walking beside participants and families with respect, care, and genuine connection.

Bronwyn
With over 30 years in the health industry and deep roots in Swan Hill, I bring a wealth of experience, local knowledge, and heartfelt commitment to my role at LiFT Supports. I’ve been part of the team for more than two years, working within NDIS guidelines to help participants connect with the supports and services they need to reach their goals.
I’m known for being well-organised, caring, and approachable, and I take great pride in building strong relationships with participants and their families. Supporting people to live life on their terms is what drives me, and I’m always happy to help.
Outside of work, I’m a proud mum of three and grandmother to a lively crew. I love spending time at the farm with family and friends, enjoying bush BBQs, water adventures, and long walks. You’ll often find me treasure hunting at antique shops or garage sales, especially if there’s fine bone china or crystal involved. My kitchen table is usually covered in jigsaw puzzles, and yes, I’m the only one in the family who can make and link gourmet sausages!
Despite claiming not to be a dog lover, my family insists I’m smitten with Elkie, my son’s cheeky Dachshund. And if you ever need a laugh, just ask about my map-reading skills… I’m just saying.

Hope
As a proud lifelong Swan Hill local and busy mum of two school-aged kids, I bring both heart and hustle to my role at LiFT Supports. With over 18 years of experience in the community services sector, including disability and mainstream employment services as well as youth work, I’m passionate about helping people live fulfilling, wholesome lives.
Since joining LiFT Supports in 2025, I’ve loved every moment of working with our participants and being part of such a passionate, supportive team. I’m known for being efficient, organised, and attentive to individual needs, always focused on achieving the best possible outcomes, no matter the situation.
What I love most about this work is the diversity. Every participant has a unique story, and I feel privileged to walk alongside them, helping to navigate challenges and celebrate progress.
Outside of work, you’ll find me experimenting with new recipes, reading mystery novels, walking my mini foxy Rosie, or coaching gymnastics at our local club. Life is full, and I wouldn’t have it any other way.
